About The Position

As an Enhanced Care Management/Community Support Case Manager, you will be responsible for providing comprehensive case management services to high need Medi Cal beneficiaries under the CalAIM initiative. This position involves coordinating care, developing individualized care plans, and facilitating access to medical, behavioral, and social services. You will work closely with patients, healthcare providers, and community resources to ensure integrated and holistic care that addresses the diverse needs of our clients. The ideal candidate will have strong clinical skills, experience in case management, and a passion for improving health outcomes for underserved populations. Excellent communication, organizational, and problem-solving abilities are essential for success in this role.

Requirements

  • Associates degree and two (2) years of experience in Mental Health or community services; or Bachelor's degree in Social Services or related field.
  • Experience working in case management, preferably with a background in healthcare or social services highly desirable.
  • Valid driver's license and proof of current auto insurance.
  • Reliable vehicle with appropriate registration and inspection for travel to client locations and other work-related duties.
  • State regulations require a physical examination, current tuberculosis test, and verified fingerprint anticriminal records clearance before beginning work.
  • Ability to read and comprehend documents such as safety rules, operating and maintenance instructions and procedure manuals.
  • Ability to respond effectively to sensitive inquiries or complaints.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of clients or employees of organization.
  • Ability to add, subtracts, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compare rate, ratio and percent.
  • Ability to understand and use a variety of computer applications and appropriate software, including spreadsheets, word processing; and workplace electronic technology, including internet access and email.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized or not standardized situation.
  • Knowledge of and sensitivity to diversity and understanding the needs of clients from different cultures and socioeconomic backgrounds.
  • Knowledge of sound medical, dental and nutritional practices for young children.
  • Awareness of community resources and services.
  • Effective organizational and time management skills.
  • Ability to problem solve and be proactive.
  • Ability to work and communicate effectively and professionally with coworkers, parents, children and members of the community.
  • Ability to travel occasionally within and outside the service area.
  • Ability to work some evenings and weekends for trainings, committee events and recruitment events.

Responsibilities

  • Conduct comprehensive assessments of clients' health, social, and environmental needs to develop personalized care plans.
  • Coordinate with healthcare providers, social services, and other community resources to ensure clients receive holistic and continuous care.
  • Regularly monitor clients' progress, adjusting care plans as necessary, and conduct follow-up visits or calls to ensure they are adhering to their care plans.
  • Advocate on behalf of clients to secure necessary services and support, ensuring their needs and preferences are met.
  • Maintain accurate and timely documentation of client interactions, care plans, and progress reports, ensuring compliance with organizational and regulatory standards.
  • Educate clients and their families about their health conditions, treatment options, and available resources to empower them to make informed decisions.
  • Work collaboratively with multidisciplinary teams to ensure integrated and coordinated care, participating in case conferences and team meetings as needed.
  • Provide immediate support and intervention in crisis situations, connecting clients with emergency resources and services.
  • Participate in training and professional development opportunities to stay current with best practices in case management and ECM protocols.
  • Plan, organize, and participate in community fairs and outreach events to promote the ECM program and its services.
  • Collaborate with marketing and outreach teams to develop materials and strategies for engaging the community effectively.
