About The Position

The Breast Imaging Center Patient Navigator's primary function is guiding high risk breast clinic patients through the healthcare system by identifying those in need of further diagnostic imaging such as six month follow up mammograms/Ultrasounds as well as screening and diagnostic Breast MRI's, and or pre Genetic counseling & testing as well as breast biopsies. They will also be helping in assisting the Breast Radiologist in delivering biopsy results. This Navigator will help the Breast Radiologist in developing relationships with service providers, and tracking interventions and outcomes from the high risk breast clinic and other activities. The Navigator can also connect patients and or families to resources like No Woman Left Behind Program as well as any social services programs or community support groups for any breast patients in need.

Requirements

Demonstrate strong public speaking and communication skills, both oral and written.
Knowledge of national Imaging Protocols, guidelines and standards of care for the management of patients with breast disease.
Exhibit strong problem-solving, priority-setting, and decision-making skills.
Manage multiple projects independently with an entrepreneurial approach.
Display excellent organizational skills with a focus on details and follow through towards completion.
Develop strong working relationships with volunteers.
Demonstrate strong computer skills, knowledge of Microsoft Office Suite, email, and internet.
Demonstrate clinical competency in patient care, teaching, and management of patients with breast health issues whether benign or malignant.

Associates Degree in Radiologic Sciences is Required OR
Associates Degree in Nursing is Required
4 years of professional experience in a Related Field is Required
IN Licensed Registered Nurse OR IN ARRT Radiologic Technologist License is Required
IN Department of Health State Radiologic Technologist Operators License is Required
CPR is Required
National Consortium of Breast Centers Breast Navigator is Required
If not already certified as a Navigator this will need to be completed within one year of hire.
Car, valid driver's license, and current automobile insurance coverage is Required

Nice To Haves

4 years of experience in Healthcare is Preferred
1 year of experience with statistical data analysis and program evaluation is Preferred

Responsibilities

Identifies high risk patients via high risk system.
Works to coordinate diagnostic imaging such as Screening/Diagnostic Breast MRI's & or genetic testing with the patient as well as coordinating with the physician office.
Works with all services to provide consistent, organized and understandable care for the diagnostic phases of a patient's treatment plan.
Facilitates interaction and communication when needed in regards to diagnostic planning with the primary care physician, consulting physicians, and hospital departments in order to enhance the patients experience and increase efficiency and effectiveness of the patient's care.
Initiate communication with patients upon learning they have a suspicious mammogram or positive finding and ensuring the patient continues to move forward with plan of action set in place along with assisting in these biopsies as well.
Track interventions and outcomes such as ensuring patients continue yearly screenings after definitive treatments such as post lumpectomy Imaging.
Provides consultation, training, and assistance to community organizations developing, implementing, and evaluating breast health screening, and or education programs.
Provides education to patients and any family members regarding their increased risk for breast cancer.
Guide patients through the healthcare system and help patients arrive at scheduled appointments on time and prepared.
Provide access and or remove barriers to support that will enable the patient to effectively cope with the impact of a breast cancer diagnosis and or subsequent lifestyle changes that might need to occur to lower their risk factors for Breast Cancer.
Offer educational sessions to inform practitioners of breast health navigator role, services provided, and encourage referrals.
Assist with referring patients with transportation where needed.
Build relationships with other patient navigators.
Assist with the High Risk Osteoporosis Clinic as needed.
Create and maintain a database of relevant breast health and breast cancer programs and services offered in the community and communicate/distribute resources to other agencies and members of the community if materials are available.
Receive, respond to, and track community resource and patient navigation calls in a timely manner.
Assist in representing the Breast & Bone Health program across the service area.
Develop ongoing, mutually beneficial relationships with key stakeholders in priority counties as identified by the community health needs assessment and participate in the action plan to remove barriers identified.
Provide information and other resources for the area's not-for-profit community, providing referrals or direct assistance as needed.
Assist in the development of new programs that address community needs and new or emerging issues related to breast & bone health per our Community Health Needs Assessment.
Provide consultation, training, and assistance to community organizations in developing, implementing, and evaluating breast health screening and/or breast and bone education programs.
Ensure the completion of a high quality community needs assessment to identify the needs and gaps in services within the Affiliate service area if any.
Integrate results of the needs assessment into all hospital centered activities if needed.
Other activities deemed appropriate by the Manager that will promote and enhance patient services throughout the Affiliate service area.
