The Blended Case Manager role offers an exciting opportunity to support youth with both physical and behavioral health needs—ensuring they receive the right services, at the right time, from the right people. The Blended Case Manager collaborates with and monitors other service deliveries to ensure the services are providing what families need. This is more than just a job—it’s a chance to be a champion for families navigating systems like education and mental health, while building strong relationships that lead to long-term success. You’ll collaborate across programs, advocate for specialized supports, and celebrate real progress every day.
