El Futuro is currently hiring a temporary, full-time, bilingual Clinical Case Manager to provide a combination of clinical case management and broker case management services for El Futuro’s Mentes Fuertes participants. The Clinical Case Manager will be responsible for evaluating and coordinating services with the Community Mental Health Workers team and other providers in the community, as well as advocating for the needs of participants to support the Social Determinants of Health of adults enrolled in the Mentes Fuertes program. This is a full-time, temporary position ending on January 31, 2027, with the possibility to transition to a permanent role contingent on continued funding. This position works 37.5 hours per week, primarily in-person, at various locations throughout Durham, Orange, Wake, and Chatham counties. The typical weekly schedule includes evenings and occasional weekends. The pay rate is $23.00 per hour. Requirements

  • Bachelor’s degree in Psychology, Social Work, Human Services OR equivalent training and experience.
  • Bilingual fluency in speaking and writing in English and Spanish.
  • Proficiency in Microsoft Office and project management systems.
  • Ability to work autonomously as well as collaboratively in a dynamic, multicultural setting.
  • Skilled in managing multiple tasks and priorities with meticulous attention to detail and organizational proficiency.
  • Demonstrated flexibility, creativity, and adaptability to cater to evolving client and organizational needs.
  • Ability to maintain professional boundaries and ensure participant confidentiality in accordance with organizational policy and applicable regulations.

Nice To Haves

  • Case Management certification.
  • Experience in a health care setting as a Case Manager or similar role.

Responsibilities

  • Facilitate care coordination between the participants, El Futuro providers, and allied community organizations who are or may need to be involved in participant's care (i.e. clinics, schools, DV/SA services, legal services). This may include scheduling appointments, facilitating communication between providers, sharing resources, and ensuring that participants receive the necessary services.
  • Maintain accurate and timely documentation of case management services provided.
  • Communicate timely participant progress, interventions, and communication with the participant’s team of care to ensure continuity of care.
  • Conduct screenings for participants.
  • Attend regular meetings with Mentes Fuertes team, supervisor, and as needed, with the Case Management Manager for additional support.
  • Research and attend allied service provider meetings related to serving the Latino community, to create and maintain positive relationships.
  • Contribute to team knowledge of community resources by providing individual consultations with Community Mental Health Workers, sharing resource updates at staff meetings and contributing to staff newsletters.
  • Participate in periodic skills-based training as directed by supervisor.
  • Conduct post surveys as needed to identify program’s strengths and areas of growth.
  • Maintain resource database, ensuring it is up to date.
  • Assist in the development and fostering of relationships with community agencies.
  • Provide ad-hoc administrative support to the Mentes Fuertes program.
  • Participate in meetings and organizational related activities as directed by the supervisor, which may include All Staff Meeting, Individual Supervision, participation on a committee, or other activities.
  • Play an active role in the continued development of El Futuro’s work via participation in fundraising and other agency-wide activities.
  • Engage in learning opportunities through “La Mesita,” El Futuro’s training platform and network of bilingual providers in North Carolina, or through other appropriate career development opportunities.
