About this position: Location: Owensboro, KY Compensation: $52,000/year Hours: First shift, 40 hours per week; on-call responsibilities. This position regularly requires flexible work hours to include evenings and weekends based on program needs. The Bilingual Case Manager acts as a primary point of contact and liaison for the youth, family, identified sponsors and external agencies. The Case Manager coordinates care for the youth on their caseload from admission to discharge. The Case Manager will engage children and families in a positive therapeutic alliance. This position requires fluency in English and Spanish (speaking, reading and writing). Client Admission: Responsible for completion of admission requirements, assessments, and creation of Individual Service Plans (ISP). Assess strengths and level of functioning of children and families and determine their needs and appropriate treatment strategies using sound, objective decision making. Program Delivery : The Case Manager coordinates the timely collection and completion of required documentation for youth in care, ensuring that the youth on his/her caseload receives the full range of services identified in the ISP in a holistic manner. Communicate regularly with KyUMH staff and community partners to address youths’ needs. Facilitates regular case management meetings for individuals, groups and families. Discharge: The Case Manager screens identified sponsors and assists them with documentation needs for youth to be discharged. Responsible for taking the lead role in making recommendations for client discharge. For this position, we require: Bachelor’s degree in behavioral sciences, human services or social services fields. If degree was obtained outside of the United States, a report from an accredited credential evaluation agency is required to provide U.S. education equivalency. Child welfare and/or case management experience is strongly encouraged. Fluency in both English and Spanish (speaking, reading and writing)
