About The Position

Join Our Team at Valley-Wide Health Systems, Inc. Valley-Wide Health Systems is seeking a compassionate and collaborative Behavioral Health Provider to deliver integrated, whole-person behavioral health care within our team-based primary care model, supporting patients’ mental, emotional, and social well-being across the lifespan. Key Responsibilities Serve as a Behavioral Health Provider (BHP) within a team-based care (TBC) model, delivering whole-person (biopsychosocial) care. Identify and assess at-risk patients, including crisis evaluation and intervention. Conduct diagnostic mental health assessments and screenings; determine level of care and refer to specialty services as needed. Develop and implement biopsychosocial treatment and relapse-prevention plans in collaboration with patients, families, and care teams. Provide evidence-based behavioral health interventions and psychotherapy. Monitor patient progress, ensure timely and accurate documentation, and communicate updates with the interdisciplinary care team. Collaborate with internal and external partners, including Enabling Services, to address social determinants of health. Participate in integrated care meetings and support the ongoing development of integrated behavioral health infrastructure. Maintain professional relationships with community agencies, healthcare partners, and local organizations.

Requirements

  • Master’s or Doctoral degree in psychology, counseling, or clinical social work.
  • Licensed or license-eligible in Colorado as a Behavioral Health Provider (LCSW, LPC, LMFT, PsyD, or PhD).
  • LPCCs must complete the Jurisprudence Exam and obtain LPC licensure within three years of hire.
  • Two years of clinical experience preferred.
  • Knowledge of mental health assessment, psychotherapy, crisis intervention, and DSM-5/ICD-10.
  • Ability to pass a behavioral health background check.
  • Valid Colorado driver’s license with ability to travel.
  • BLS certification provided upon hire.
