About The Position

Implements the treatment plan in collaboration with the treatment team. Provides a variety of peer support, advocacy, and other recovery services to clients.

Requirements

  • High School Diploma or equivalent required.
  • Current Certified Recovery Peer Advocate or Certified Recovery Peer Advocate - Provisional certification preferred.
  • 1-3 years of relevant experience, required.

Responsibilities

  • Collaborates with the treatment team to facilitate implementation of the treatment plan.
  • Provides warm hand-off to participants prior to discharge from hospital to outpatient services.
  • Provides a variety of peer support, advocacy, and other recovery services to clients and caregivers.
  • Develops a relationship of trust with each participant, functioning as a role model, peer support, mentor, advocate, and ally.
  • Communicates hope and encouragement to assist participants in developing readiness to discharge from the hospital and return to their community.
  • Provides case management service to identified clients while maintaining the wellbeing of clients and integrity of the program.
  • Offers assistance with independent living skills, and community adjustment, helping the individual make a positive connection to outpatient behavioral health services and other community resources.
