About The Position

Provides support and follow-up to clients enrolled in the MRT program by using lived experience to promote healing, connection, and successful engagement in services and community resources.

Requirements

  • High School Diploma or equivalent required.
  • Prior experience working with individuals affected by substance use disorder (SUD), mental health conditions, or criminal justice involvement.
  • Willingness to share and disclose relevant lived experiences to promote recovery.
  • Proficiency in Microsoft Word required.
  • Ability to navigate New York City public transportation independently.
  • Excellent verbal and written communication skills; ability to engage respectfully and effectively with individuals and groups regarding complex and sensitive issues.
  • Physical ability to perform job functions including walking, escorting, and assisting clients in community settings.

Nice To Haves

  • Familiarity with the Foothold EHR system preferred.
  • Bilingual in Spanish/English strongly preferred.

Responsibilities

  • Provide support to clients by sharing lived experience and promoting hope and self- determination.
  • Document all services provided in the Foothold Electronic Health Record (EHR) system.
  • Attend weekly supervision meetings with the Program Director
  • Escort clients to medical, mental health, social service (e.g., HRA), and other critical appointments as needed. Offer emotional support, translation, or interpretation assistance to clients during appointments.
  • Provide support to clients experiencing pre-crisis or crisis situations, along with the MRT team.
  • Teach and model successful housing and community integration skills.
  • Share accurate and helpful information on public benefits, entitlements, and other forms of support.
  • Conduct outreach to connect clients with desired services and ensure continuity of care.
  • Link participants to appropriate community-based resources and treatment providers.
  • Provide peer mentoring, co-facilitate support groups, and train clients in Narcan (Naloxone) administration.
  • Actively engage and support clients participating in recovery treatment
  • Provide administrative support to the MRT team, including processing and tracking novices, check requisition, as well as reporting and chart management in accordance with organizational policies.
  • Compile, verify, and submit invoices to the Accounts Payable department.
