Requirements
- Graduate of an accredited nursing program (BSN preferred).
- Current California Registered Nurse (RN) license in good standing.
- Minimum of 1 year of professional psychiatric or behavioral health nursing experience (home health or community-based setting preferred).
- Strong clinical judgment, therapeutic communication, and crisis-intervention skills.
- Valid driver’s license, reliable transportation, and ability to travel to patient homes in your assigned geopod.
Nice To Haves
- Public Health Nurse (PHN) certification a plus.
Responsibilities
- Conduct in-home psychiatric nursing assessments, including evaluation of mental status, psychosocial factors, and safety risks.
- Provide skilled interventions to manage symptoms of mental illness, including medication administration, teaching, and monitoring.
- Educate patients and families on mental health conditions, coping strategies, and treatment adherence.
- Develop and implement individualized care plans focused on behavioral health needs while coordinating with the interdisciplinary team.
- Collaborate closely with physicians, therapists, social workers, and the Director of Nursing to ensure continuity of care.
- Document patient progress and update care plans as conditions evolve.
