Requirements

  • Graduate of an accredited nursing program (BSN preferred).
  • Current California Registered Nurse (RN) license in good standing.
  • Minimum of 1 year of professional psychiatric or behavioral health nursing experience (home health or community-based setting preferred).
  • Strong clinical judgment, therapeutic communication, and crisis-intervention skills.
  • Valid driver’s license, reliable transportation, and ability to travel to patient homes in your assigned geopod.

Nice To Haves

  • Public Health Nurse (PHN) certification a plus.

Responsibilities

  • Conduct in-home psychiatric nursing assessments, including evaluation of mental status, psychosocial factors, and safety risks.
  • Provide skilled interventions to manage symptoms of mental illness, including medication administration, teaching, and monitoring.
  • Educate patients and families on mental health conditions, coping strategies, and treatment adherence.
  • Develop and implement individualized care plans focused on behavioral health needs while coordinating with the interdisciplinary team.
  • Collaborate closely with physicians, therapists, social workers, and the Director of Nursing to ensure continuity of care.
  • Document patient progress and update care plans as conditions evolve.
