If you’re driven to help others thrive and want to be part of a collaborative, mission-focused team, this is your chance.   We are seeking a Behavioral Health Navigator to join our community-based team in Colmar, PA, serving clients across Montgomery County. In this role, you’ll use your clinical expertise to support individuals with mental health needs and help integrate behavioral health into primary care—ensuring every person receives the comprehensive support they deserve.

Requirements

  • Master's degree in social work, Psychology, Nursing, Counseling, or related field of study.
  • 1 year minimum of direct mental health care experience required.
  • A valid Driver's license, clean driving record with reliable vehicle is required.
  • Independent computer skills with Microsoft office and EHR
  • Able to work Monday-Friday 8:30am-4:30pm
  • This role is community-based so it requires travel to various schools, homes, and other community locations to provide services and support clients. Mileage reimbursement is provided.

Nice To Haves

  • Experience and training in Motivational Interviewing, co-occurring disorders, trauma informed care, and integrated physical health/behavioral health care, preferred.

Responsibilities

  • Providing initial and on-going assessment of consumer’s mental health, physical health, and substance use and response to treatment
  • Collaborating and communicating with other healthcare staff involved in the individual’s’ care. Complete and coordinate necessary referrals. Provide follow-up to ensure recommended services are provided.
  • Coordinating with community and inpatient PH and BH professionals to ensure consumer status is shared upon inpatient admission and discharge
  • Educate and support the use of self-management strategies to increase the participation of the person in their own health care.
  • Assisting the individual in obtaining and maintaining basic living needs and skills including housing, food, medical care, recreation, education, and employment. If additional support is needed, make recommendations and referrals.
  • Completing all needed documentation as per program guidelines and payor requirements. This includes all assessment, releases of information, consents, progress notes, wellness plans.
  • Coordination with county MH, IDD, D&A, Office of Aging, and any other applicable resources.
