About The Position

Sentara Health Plans in Richmond is looking to hire a Remote Behavioral Health Care Coordinator. This position is remote, however, candidates must reside in the State of Virginia. The Remote Behavioral Health Care Coordinator works in a collaborative role with all members of the health care team and community service agencies to coordinate Behavioral Health and PUMS program for Optima Family Care members to improve health and safety outcomes. Assesses for barriers to compliance with the medical care plan and implements appropriate interventions to promote compliance with recommended therapy. Acts as a resource to assist members in need of community-based programs for Mental Health and Substance Abuse. Empowers the member with the skills to navigate the process for obtaining appropriate medical care and community assistance, with a focus on members with a history of Medication or Emergency Department inappropriate utilization, to include substance abuse, misuse, and chronic pain and/or mental health issues. Works with Pharmacy Department to restrict pharmacy providers and Medical Providers, for prescription services for PUMS members.

Requirements

  • Bachelor's degree in social work, Health and Human Services or degree in related field with 15 credit hours dedicated to health and human development required.
  • 3 years of related experience in a physician office setting, Community Services Board, acute care or in a managed care setting.
  • Working directly with patients related to Behavioral Health or Substance Abuse treatment is required.
