About The Position

The Spirit of Healing Behavioral Health Care Coordinator works independently and collaboratively with a team of health professionals to provide members with opportunities to improve self-management abilities, health outcomes, and satisfaction with health care in a culturally specific manner. This position works collaboratively with American Indian or Alaskan Native members who may have complex medical/psychosocial/ addiction issues and their families/caregivers as well as with members of multidisciplinary teams (MDT) to provide care coordination and mental health assessments as needed.

Requirements

  • Master’s Degree in Counseling Psychology, Social Work, Marriage and Family Counseling, or related clinical field
  • Minimum 2 years’ experience in behavioral health, care management, and care coordination and/or social services.
  • Active Oregon license (LCSW or LPC) or ability to obtain by start date
  • Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
  • Ability to operate a motorized vehicle
  • Demonstrated skill in word processing and computer skills and ability to learn new technological means of communication and various data base
  • Ability to maintain professional boundaries with clients, the community, and other staff members.
  • Must have two years of experience using an electronic health record
  • Ability to pass a pre-employment or for cause drug tests.
  • Ability to pass criminal background and DMV checks.

  • Experience working with Native American/ Alaska Native community is considered a plus.

Responsibilities

  • Actively collaborate and coordinate Care Coordination for Spirit of Healing Members/NARA NW patients utilizing a team approach
  • Administer mental health assessments and treatment plans as needed to streamline member into services
  • Provide timely updates regarding the plan of care that includes self-management as well as member specific goals.
  • Makes referrals to community programs/services as appropriate.
  • Utilize the Electronic Medical Records system for documentation.
  • Make independent decisions regarding client care and care management.
  • Complete appropriate documentation in a timely manner.
  • Meet with members to identify and reduce or eliminate barriers to care (e.g., social, cultural, linguistic, education), and set effective goals with members.
  • Utilize a culturally sensitive person-centered approach in working with clients by respecting their self-determination and autonomy and exploring their ambivalence by utilizing Motivational Interviewing principles.
  • Ensure that clients get timely access to appropriate services by promoting and modeling advocacy skills.
  • Collaborate with medical and auxiliary providers in the community to develop health care plans/goals based on identified needs and employ creative strategies to address psychosocial issues.
  • Facilitate communication and positive relationship building between clients and medical providers; attend medical appointments as needed.
