About The Position

Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Your Goal: The BBO-C Care Navigator monitors, assesses, evaluates, educates, and provides intervention and referrals to facilitate coordination of medical health, oral health, and social services for low-income families. How You’ll Help Patients: Accurately documents client activity in Cornerstone. Monitors and assesses client/family needs. Provides care navigation in accordance with the standards of the BBO-C program. Schedules and performs home visits for clients as needed. Performs Ages and Stages Questionnaires with applicable clients. Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor. Engages in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers. Requirements

  • High school diploma or general education degree (GED) required.
  • Associate or bachelor’s degree in health or social services field or 3 years of equivalent work experience in health care preferred.

Nice To Haves

  • Medical Assistant, Certified Nursing Assistant, or Community Health Worker experience is preferred.
