About The Position

Reviews the patient’s needs to determine whether the individual meets the program’s parameters. Coordinates all bariatric services provided in association with the program’s patients. Follows the patient throughout the steps of the program. Ensures patients receive appropriate psychological and other counseling. Facilitates and schedules support groups and educational events. Communicate with service delivery partners, physicians, and other health professionals to provide case coordination and develop a plan of care that facilitates the efficient use of health care resources. Effectively assess, coach and graduate patients from care, resulting in successful patient outcomes. Communicates with other healthcare professionals as appropriate, informing them of patient’s progress and/or any potential concerns. Coordinate and document all details of patient’s progress throughout and after program participation. Facilitating/leading the hospital bariatric program Multidisciplinary Task Force, ensuring coordinated comprehensive program that includes nurses, dietitians and physician extenders. Develop and maintain bariatric partnerships with outlying hospitals to assist patients to get surgery at Community Hospital and have pre and post-operative care and support done in home community as needed.

Requirements

  • High School diploma or equivalent; Associates Degree or Bachelor’s Degree in healthcare preferred.
  • Previous proven experience and expertise in Surgery scheduling, insurance authorizations, and pre-surgical testing with a minimum five (5) years in healthcare services preferred OR equivalent combination of education and experience.
  • Must relocate to Grand Junction, CO 81505 before starting work.
