About The Position

Patient Navigator Coordinator The Patient Navigation Coordinator provides individualized assistance to patients, families, and caregivers to overcome healthcare system barriers and access resources throughout the cancer care continuum—from prevention to end-of-life care. Under the guidance of Social Workers, this role supports patients with providing resources, education materials, form completion, and health-related screenings while ensuring compliance with all standards and regulations. The general pay scale for this position at Willamette Valley Cancer Institute and Research Center (WVCI) is $22.00 - $30.24. The actual hiring rate is dependent on many factors, including but not limited to prior work experience, education, job/position responsibilities, location, work performance, etc. Employment Type: Full Time, 1.0 FTE Benefits: M/D/V, Life Ins., 401(k) Location: Eugene, OR

Requirements

  • High school diploma or GED; advanced education preferred.
  • A minimum of four (4) years of experience in a clinical health care environment is required.

Nice To Haves

  • Medical Assistant (certified) experience preferred.
  • Health care-related education, planning, management, or similar experience is preferred.

Responsibilities

  • Connects patients with resources and services for practical needs.
  • Communicating with physicians, advanced practice providers, nurses, social workers, and other members of the care team.
  • Transfer/escalate situations when appropriate with accurate information utilizing resources to resolve inquiries within the scope of the department.
  • Travel intermittently to Northern Clinics in Albany and Corvallis for in-person patient support.
  • Document patient interactions and services in the EMR system.
  • Act as a liaison and advocate for patients and caregivers.
  • Participate in process improvement initiatives to enhance patient outcomes and satisfaction.
  • Maintain strict patient confidentiality in compliance with HIPAA and organizational policies.
  • Perform other duties as assigned.
