About The Position

Asociación Puertorriqueños en Marcha (APM) has been helping families achieve their greatest potential since 1970 by providing early childhood education, foster care & adoption, child welfare, mental & behavioral health, community and economic development, housing, and community school services to the North Philadelphia communities. A focus on services that ensure household stability. These services include but are not limited to providing direct connection to resources for parents and families, hosting Family Forums at the school and nearby for the school community, and working with the School Coordinator to better support the School Advisory Council and survey and needs assessment processes.

Requirements

  • Master's Degree in Social Work or a closely related field
  • Two (2) years of specific social service work experience (i.e., related fields are social services, sociology, psychology, theology, education, criminal justice, human services, and public administration).
  • FBI fingerprints, PA child abuse clearance, and PA criminal history background check.
  • Able to work evening and weekends when necessary.
  • Strong clinical writing skills
  • Strong professional communication skills
  • May be required to use own vehicle or public transportation when other means of transportation are unavailable.
  • Must demonstrate genuine empathy and concern for individuals as indicated in our corporate vision, mission and values statements.
  • Knowledge of community resources and ability to work as a team member
  • Adhere to applicable city, county, state and federal laws and regulations, internal and DPW licensing standards.

Nice To Haves

  • Familiarity in computers including word processing programs and internet.
  • Knowledge of social work values and/or child welfare services preferred.

Responsibilities

  • Develop a mastery proficiency of Attendance Case Manager program procedures, policies and operations.
  • Facilitate peer review and supervision meetings with staff to address the underlying causes of chronic absenteeism, poor academic performance and family instability, promote the safety and well-being of families, and highlight social work best practices.
  • Ensure that mandated reporting responsibilities are met and minimum requirements of case management services are completed with families.
  • Assist with direct services to families through phone calls, in-person home visits, and participation in meetings with clients.
  • Monitor outreach, documentation, and data entry for all students in a service period. Support school relationship building with the School Coordinator, School Advisory Committee, students, family members and community providers.
  • Support children and families in a manner that fosters independence, growth and development and will facilitate the child and families achieving their stated goals.
  • Assess the strengths, supports, and requirements of children, families and communities in which they live.
  • Ensure that referrals are completed promptly and confirm that a linkage between the family and provider is secured.
  • Monitor and enter data into the OCF Database, and APM data collection methodology.
  • Identify staff professional development needs. Develop professional development materials and facilitate in-house training sessions. Recruit external organizations to lead trainings.
  • Keep accurate written records of all biweekly supervisory meetings. Ensure that all monthly home and school visits are performed and documented. Submit supervision reports and accompanying documents on a monthly basis.
  • Plan, track, complete and accurately maintain records on file audits, database review activities, and caseload assignments of all Case Managers within set program deadlines and contract requirements.
  • Assist with recruitment and hiring efforts, train and on-board new hires, monitor staff performance, conduct performance evaluations, complete correction action plans, and approve timesheets.
  • Maintain consistent communication with staff, the administrative team, management team, and program partners. Develop knowledge of all organizational policies and procedures.
  • Participate in case conferences, school conferences, all teaming conferences deemed necessary to support movement of the case.
  • Perform other duties that support the mission of the Case Management Service in Community Schools program and APM mission and vision.
