About The Position

What is an OhioRISE? The Ohio Department of Medicaid (ODM) is committed to improving the health of Ohioans and strengthening communities and families through quality care. In 2020, ODM introduced a new vision for Ohio's Medicaid program - one that strengthens Ohio's future and ensures everyone has the chance to live life to its full potential. OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio’s first highly integrated care program for youth with complex behavioral health and multi-system needs. Care Management Entities (CMEs) are vital to the success of the OhioRISE model and will each serve a separate catchment area – part of a county or multiple counties that make up their geographic footprint. Summary: Utilizing high fidelity wraparound services, OhioRISE child and family teams provide moderate and intensive care coordination for Ohio’s most behaviorally complex children. This position oversees the delivery of clinical support that is provided to care coordinators who facilitate the planning of complex clinical services for TVN and its subdelegates.

Requirements

  • LSW or PC licensure required; Independent licensure preferred
  • Must complete the high-fidelity wraparound training program provided by the Cener of Excellence, an entity recognized by ODM.
  • A minimum of 2 years of supervisory experience in the OhioRISE Program
  • Be culturally competent or responsive with training and experience necessary to manage complex cases.
  • Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).
  • Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
  • Willingness to travel for various reasons, mainly during the day but occasionally overnight.
  • Ability to visit clients’ homes (may or may not be handicapped accessible).
  • Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
  • Maintain a HIPPA compliant home-based office space

Responsibilities

  • Facilitates the process for clinical management and escalation for members who demonstrate high acuity needs; This includes but is not limited to identifying complex members with emergent needs, serving as a consultant for child and family teams to explore and expand options for supporting members with high needs in the community, and determining timing for escalating supports to Aetna/ODM
  • Conduct internal reviews and ensure care/safety plans are in place to support members with complex needs; Assure clear action plans are created to support Youth with High Acuity Needs; Work directly with supervisor and care coordinator to support updates and to create back up plans when treatment options are not immediately available;
  • Oversee transition and discharge planning
  • Represent leadership for the TVN CME in various meetings including but not limited to: child and family team meetings, clinical escalation meetings, PRTF Collaboration meetings, Inpatient Clinical Reviews, MSY Operational Meetings, community stakeholder meetings, etc.
  • Collect, analyze and report data related to Youth with High Acuity in alignment with contractual requirements for monthly metrics. Assure documentation standards are met in alignment with program requirements
  • Facilitate monthly meetings with OhioRISE supervisors for training, consultation, and feedback
  • Provide direct supervision and training to care coordinators and supervisors, as needed
  • Other duties as assigned
