About The Position

Works under the supervision of the Nurse Manager of Hospice Outpatient. The Assistant Nurse Manager of Hospice Outpatient is responsible for the implementation, delivery, coordination and supervision of hospice services delivered by their respective team that is consistent with department policies and procedures. Performs other duties as assigned.

Requirements

  • High School Diploma or successful completion of an equivalent High School Exam Required
  • BSN required
  • Licensed to practice as an RN in Ohio required
  • BLS within 90 days of hire required
  • BMVL required (Must maintain eligibility to drive as determined by SOMC insurance carriers)
  • Approximately two years as a Registered Nurse with experience working with terminally ill patients in a home environment preferred.

Responsibilities

  • Supervises the planning, coordination and delivery of the interdisciplinary team interventions.
  • Provides, delegates, and coordinates patient care.
  • Assigns patient/family to appropriate staff member(s).
  • Participates and provides leadership in the team for regular interdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solving sessions; and delivery of team support.
  • Implement and monitor compliance with program policies and procedures related clinical services.
  • Serve as a member of the clinical management team in order to address problems associated with the delivery of clinical services and staff development needs, to put forth suggestions for appropriate additions/changes in policies and procedures, and to give peer input to enhance and improve the middle management function.
  • Supervise the maintenance of clinical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, Joint Commission standards, legal and ethical standards.
  • Provide supervision of support and clerical staff in cooperation with other members of the clinical management team.
  • Oversee clinical supervision of nursing personnel and home health aides and conduct written performance evaluations.
  • Conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members.
  • Provide leadership and counsel to nursing personnel to measurably enhance staff development.
  • Identify need for staff education and assist in the development of in-service programs for the interdisciplinary team.
  • Participate in staff selection and orientation of new personnel as appropriate to team responsibilities.
  • Perform specific assignments as directed by the Outpatient Nurse Manager.
  • Pre-cleans, decontaminates and transports contaminated instruments according to policy
  • Performs intermediate-level disinfection and/ or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU's
  • Performs other duties as assigned.
