About The Position

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. We are currently seeking a Registered Nurse to serve as a Hospice Nurse Case Manager. This person will be responsible for the provision of skilled nursing assessment, planning and care to maximize the comfort and health of patients and families. Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Current license as a registered professional nurse in the State of California.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred but not required.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Responsibilities

  • Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians.
  • Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.
