Middlefield Oaks Senior Living, located in Cottage Grove, Oregon, is seeking a supportive and experienced Assistant Health and Wellness Director to help oversee our Health Services program and ensure high‑quality care is being provided to the residents in our community. Requirements

  • High school diploma or equivalent.
  • Current nursing license in the state of employment.
  • At least one (1) year of prior nursing experience in a senior living community or related field preferred.
  • Basic computer skills.
  • Ability to speak, read, and write in English.
  • Ability to obtain continuing education credits as required by state regulations.
  • All candidates must be able to pass a criminal background check and a comprehensive reference check.

Nice To Haves

  • At least one (1) year of prior nursing experience in a senior living community or related field preferred.

Responsibilities

  • Supports the Health and Wellness Director in leading the Health Services team and communicating professionally with employees, residents, families/responsible parties, and other healthcare professionals.
  • Assists with the supervision, provision, and quality of nursing and supportive services within the community.
  • Performs resident assessments and helps identify needs related to physiological, psychological, environmental, and social well‑being upon move‑in and during changes of condition.
  • Assists in ensuring routine resident assessments are completed quarterly.
  • Helps ensure resident safety needs are met using appropriate assessment and evaluation tools.
  • Participates in the development and updating of care plans.
  • Supports communication with families/responsible parties, physicians, the Executive Director, and State agencies as needed.
  • Assists in investigating incidents and behavior changes requiring licensed nurse assessment and documents findings in resident progress notes.
  • Performs medication room and medication cart audits as assigned.
  • Helps monitor the 24‑hour communication and alert system for compliance, including timely assessments of residents placed on alert status.
  • Reviews physician transcriptions, progress notes, discharge orders, move‑in orders, and hospital notes as directed.
  • Supports the Executive Director and Health and Wellness Director in ensuring policies, procedures, and state regulations are followed.
  • Assists with hiring, training, and evaluating health services employees, ensuring direct care staff meet state‑required training standards.
  • Participates in new employee orientation and monthly in‑services.
  • Help with staff scheduling to ensure adequate coverage, supply purchasing, and budgetary support.
  • Ensures residents are treated with privacy, respect, and dignity.
  • Follows standard precautions and infection control procedures.
  • Completes state‑required training upon hire and annually.
