About The Position

CURRENTLY SEEKING QUALIFIED PROFESSIONAL TO JOIN OUR TEAM! Primary Job Purpose: Designated by Administration to have the authority, responsibility, and accountability for the functions, activities, and training of the Nursing team in conjunction with the Director of Nursing. Develops and maintains nursing service objectives, executes neighbor care policies, maintains standards of nursing practice and departmental fiscal responsibility, and collaborate with the CNA Supervisor and Director of Nursing and training for the nursing team while directing the ongoing quality improvement programs.

Requirements

BSN preferred, Must be currently licensed in the state of Colorado as RN or LPN, LPN applicants must be able to obtain RN licensure in the State of Colorado within 6 months of hire date
Ability to manage time and resources effectively.
Ability to communicate effectively with co-workers
Ability to work and converse in a polite manner with neighbors, their families, co-workers, and others associated with the position on a daily basis.
Ability to understand and follow verbal and written instructions

Ability to perform basic computer functions and to learn new programs as used by the facility.
Must be able to stand and walk 5-7 hours a day.
On office days, must be able to sit up to 4-8 hours.
Must be able to lift and carry 0-50 pounds by self regularly and 50+ pounds occasionally with assistance.

Nice To Haves

5 years supervisory experience in the long-term care field, preferred.
Education and training in the field of geriatrics.
Occasionally will lift neighbors with assistance.

Responsibilities

Practice the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.
Determines type and amount of nursing care needed to meet the neighbors needs prior to admission and is involved in any critical decisions involving neighbor care and monitors the admission process.
Performs daily rounds and reviews nursing reports to keep current with neighbor care and needs. (Evaluates neighbor needs and correlates care by scheduling daily rounds, reviewing nursing reports & medical records, visiting neighbors, and observing care).
Assigns responsibility and delegates’ authority for neighbor care.
Continually upgrades the care given through education and maintenance of good communication between neighbors and team members.
Participates in discharge planning. Ensures that physician orders are obtained for discharge and prepares neighbor discharge instructions.
Cooperates with all departments to maintain a safe and sanitary environment.
Manages nursing department budget and reviews monthly departmental financials with NHA and submits variance reports, in absence of the Director of Nursing.
Reviews potential admissions as part of the admissions team.
Develop and implement an ongoing infection prevention and infection control program to prevent, recognize, and control the onset and spread of infections in order to provide a safe, sanitary, and comfortable home.
Establish campus-wide systems for the prevention, identification, reporting, investigation, surveillance and control of infections and communicable diseases of neighbors, staff, and visitors.
Develop and implement written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control.
Direct and oversee the antibiotic stewardship program for the care center.
Perform analysis of collected data and use this information for infection control efforts, to detect outbreaks, direct in-service education and identify individual neighbor problems for intervention.
Report infection prevention data and improvement efforts monthly to the Continuous Quality Improvement committee.
Oversee care activities that increase the risk of infection (i.e., catheter use, wound care, skin care, incontinence care, etc.).
Provide education to all staff to assist in preventing the spread of infection on the campus; such as hand hygiene, PPE use, etc.
Implement and provide neighbor and employee vaccination programs.
Review and revise the infection prevention program, it’s standards, policies and procedures, at least annually and as needed for changes in the annual assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections.
Ensure public health, Medical Director, Director of Nursing, and Nursing Home Administrator are notified of any incidence of a reportable disease, infestation or an outbreak on the campus.
Manage restorative program and staff.
Sets standards for qualifications of personnel while following regulations regarding licensed personnel.
Responsible for hiring adequate nursing staff.
Initiates special focus groups and committees, as necessary to address neighbor care needs.
Supervises new team member orientation, supervises and evaluates team members and holds them accountable to written performance standards, in the absence of the D.O.N.
Manages performance issues of nursing team members, including counseling, retraining and performance improvement plans. Works with Human Resources and/or the Director of Nursing to terminate team members if necessary
Develops nursing goals and objectives, policies and procedures, and quality assurance measures.
Encourages continuous development for all team members to maintain and upgrade their skills and knowledge.
Promotes teamwork within departments.
Acts on behalf of the Director of Nursing in his/her absence.
Participates in medication destruction per state regulations.
Supervises duties of the nursing team members, keeping daily work schedules and maintaining adequate staffing patterns.
Is responsible for scheduling the RN’s, LPN’s and other nursing positions as needed within the HRIS system and on daily schedules.
Is responsible for assisting the C.N.A. supervisor with scheduling duties as needed and in his/her absence.
Maintains current knowledge of state and federal regulations and oversees implementation of new requirements in areas of deficient practice.
Participates in activities of committees such as CQI/QAPI, Safety, Psychotropic Review, Continued Stay Review, Medicare Team, and/or appoints other nursing personnel to appropriate committees.
Investigates allegations of abuse and substandard care, reports and manages correspondence with the Colorado Department of Public Health & Environment.
Utilizes services of outside consultants i.e.; nursing, pharmacy, and psychologists, to audit regulation compliance and guide improvement plans.
Must maintain confidentiality of all work-related information according to HIPAA regulations.
Maintains cordial relations with neighbors, families, friends, as well as individuals and groups within the community.
Promotes effective working relations with administration, medical staff, affiliated agencies, volunteers, and other departments.
Participate in the organizational assessment at least annually, for determining the knowledge and skills required among staff to meet the needs of the neighbors campus wide.
Work closely with the Nursing Home Administrator, Assisted Living Administrator and other leaders in developing a training plan that addresses all regulatory requirements and any other topics identified in the assessment.
Develop educational calendars in accordance with the plan.
Plan and schedule trainings with flexibility to ensure limited disruption of care. Post notices of trainings in advance.
Maintain all training records including attendance, competency tracking, educational materials and speaker qualifications for each educational program, including Dementia training.
Continually assesses education programs to ensure the information reflects current standards of practice and organizational policies and procedures
Collaborate with department managers for developing competencies required for meeting neighbor needs. Assist with evaluating competencies, depending on the knowledge based technical or behavioral nature of the competency.
Coordinate and schedules CPR instruction per staff needs.
Plan, schedule and execute required education days and skills fair.
Oversee the Mentor Program.
Coordinate and schedule the nurse aide training program and medication aide training program. Submit necessary information to the state agency as required.
