Requirements
- High school diploma
- Valid drivers’ license
Responsibilities
- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
- Complete patient assessments of need
- Support homecare operations within the community
- Act as the face of the organization while in the community
- Provide care insights to patients and caregivers while in the field
- Timely complete records in accordance with Company policy
