What you’ll do: Loyola University Medical Center is seeking a skilled and compassionate Advanced Practice Nurse (APN) to join our Heart Failure Program. This role provides advanced nursing care in an acute care setting, focusing on a specialized patient population. The APN will work both independently and collaboratively with physicians, case managers, and the healthcare team to ensure comprehensive, high-quality care.
Requirements
- CPR – Basic Life Support
- ACLS – Advanced Cardiovascular Life Support
- Nursing – Current Registered Nurse License - State of Illinois (RN)
- Nursing – Nursing Practitioner License – State of Illinois (APN)
- Physician Assistant License – State of Illinois (PA)
Responsibilities
- Deliver expert care to heart failure patients, promoting healing and preventing illness and hospital readmission.
- Assess patients’ health status through comprehensive history-taking, physical exams, and psychosocial evaluations.
- Develop, implement, and modify individualized treatment plans based on patient needs and clinical data.
- Perform specialized procedures and interventions within the scope of practice.
- Coordinate care transitions across service units to optimize patient outcomes.
- Educate patients and families, empowering them to participate in their healthcare decisions.
- Maintain high standards of practice in alignment with Loyola University Medical Center’s mission and care models.
