Requirements

• Master’s Degree in Nursing
• One (1) year experience.
• Please refer to the Medical Staff Guidelines for certifications and credentialing applicable to this position.
• Requires Heart Failure APP experience

Responsibilities

• Provides oversight to assigned Nurse Practitioners.
• Performs history and physical examinations and carries out appropriate intervention/procedures as determined by the plan of care.
• Coordinates care of assigned patients according to Northside Hospital established Standards of Care.
• The Lead APP will work 50% of the time with the HF Program Medical Director.
• Lead will have 12+ direct reports and will be adding 5-7 more APPs over 3-5 years.
