About The Position

The ambulatory care pharmacist is responsible for optimizing safe and effective medication use for all patients encountered in the ambulatory practice. Routine responsibilities include: comprehensive medication review, providing disease state management (diabetes, hypertension, hyperlipidemia, smoking cessation, etc.), patient recruitment for pharmacy services, answering drug information requests, optimizing medication costs, education to other healthcare professionals, and management activities related to ambulatory services.

Requirements

  • Bachelor’s or Doctor of Pharmacy degree from an ACPE accredited College of pharmacy.
  • PGY1 residency training and/or advanced certification (e.g. board certification, certified geriatric pharmacist, etc) required.
  • Licensed Pharmacist in good standing with the state of Michigan or license eligible.
  • Basic word processing skills and able to operate hospital programs used in the Pharmacy.
  • Regularly communicates with patients, families, physicians, healthcare employees from other departments and the community; exchanges factual matters related medications such use, doses, side effects, and precautions.
  • Counsels and educates and educates physicians, nurses, and the community regarding highly technical pharmaceutical information.
  • Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time.
  • Involves considerable standing or walking, regular lifting of light-weight objects (i.e., 20 pounds or less) and assisting with heavier tasks such as assisting in lifting and moving patients.
  • Minor straining or fatiguing positions must be assumed, and some fatiguing physical motions are required.

Nice To Haves

  • PGY-2 residency or fellowship training in ambulatory care or related field is preferred.
  • Board Certification is preferred.

Responsibilities

  • Works under collaborative practice agreement to conduct disease state management and comprehensive medication reviews for primary care patients.
  • Communicates effectively with patients in developing care plans that are patient-centered, cost-effective, and evidence-based.
  • Works collaboratively with physicians, mid-level practitioners, medical assistants, social workers, nurses, and office staff to optimize patient care.
  • Consults patient drug profile for allergies or drug interactions, refill status, and pricing.
  • Provides technical/clinical information and troubleshoots problem situations on drugs, dosages, contraindications, and so forth to physicians, nurses, other healthcare employees, and to the community.
  • Records all patient prescription data on patient profile; updates profile with additional orders, allergic reactions, and diagnoses.
  • Identifies any apparent conflicts within the patient's profile or drug therapy program.
  • May provide pharmacokinetic evaluation and dosing of patients on specific medications and monitors patient progress.
  • Evaluates and acts on laboratory data and patient response.
  • Maintains an up-to-date knowledge of developments and advances in the pharmacy field; fulfills continuing education requirements in order to maintain registration.
  • Maintains work area and equipment in a clean and orderly condition.
  • Answers the phone, provides general information and specific patient data as appropriate.
  • Participates in drug utilization reviews and quality assurance activities.
