About The Position

Hartford HealthCare’s Pharmacy System has grown to include 9 inpatient pharmacies, 9 cancer center infusion pharmacies, 3 retail pharmacies, and our Ambulatory Care pharmacy department supporting more than 50 medical office practices. Collaboration between our numerous pharmacy services and other members of the health care team is key to our commitment to continually improving how we provide patient care. We work hard to support our pharmacists in their efforts to optimize safe, effective, and efficient pharmaceutical care for all the people we serve. We provide our pharmacists with the tools they need to be at their best, including access to advanced resources and state-of-the-art pharmacy software. Pharmacists can advance their careers both within their positions via our career ladder as well as movement into specialized and leadership roles. The Pharmacy Department at Hartford Hospital utilizes a decentralized model with pharmacists embedded on medical teams to advice on optimal drug therapy. We also have an American Society of Health-System Pharmacists (ASHP) accredited Pharmacy Residency program, an active Oncology infusion service, and a Retail/Specialty outpatient pharmacy. The Ambulatory Care Pharmacy Specialist will collaborate as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational, safe, evidence-based and cost-effective therapy. In addition to direct patient care responsibilities, the clinical pharmacy specialist will: actively participate in committees and /or workgroups; collaborate in the review of new and existing medications, clinical practice guidelines and other related policies used in the management of the clinic-specific medical conditions. This person also conducts clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.

Requirements

  • B.S or Pharm.D. Required. PharmD preferred.
  • Current CT Pharmacist license or eligibility within 45 days
  • PGY- 1 Pharmacy residency preferred
  • Excellent customer service and communications skills
  • Prior experience with EPIC systems preferred.
