About The Position

Aging Services Assessment Specialist - Challenges Options in Aging, New Castle, PA At Catholic Charities, Diocese of Pittsburgh, we’re seeking a compassionate, supportive Aging Assessment Specialist to join our Area Agency on Aging team to bring encouragement to older adults in Lawrence County who need help with life’s challenges. With your education and experience you know the importance of listening closely and working carefully to meet client's needs and help them access the services they need. You’re in a key position, facilitating much-needed relief to members of a vulnerable population. You have a patient heart and enthusiasm to serve the geriatric population in Lawrence County. Requirements

  • A bachelor's degree in social work or a health related field required.
  • Excellent oral and written communication skills, including interviewing and assessment techniques, are required.
  • Knowledge of aging process and geriatric issues.is required.
  • The ability to interact constructively with community agencies and resources is required.
  • The ability to travel daily throughout Lawrence County is required.
  • A valid driver's license.is required.
  • Act 33/34 clearances are required.

Nice To Haves

  • A current RN license is highly desirable.
  • Previous experience performing assessments of participants' functional ability is preferred.

Responsibilities

  • Provides assessments within established timeframes, including a comprehensive review of medical documentation, for older adults to determine appropriate clinical level of care.
  • Completes records to assure appropriate compliance with PA Department of Aging (PDA)/Office of Long-Term Living (OLTL) program requirements.
  • Educates participants and provides information on all long-term care options, including community-based services that may allow participants to stay in their home.
  • Maintains confidentiality in communication with participant, staff, and others to preserve participant and staff rights.
  • Contributes to professional development through participation in elective and/or required educational programs sponsored by Area Agency on Aging, Catholic Charities, community, and Pennsylvania Department of Aging.
  • Representing the organization with community and government agencies.
  • Prepares/maintains monthly recording forms and other documents to provide current, accurate program and service data.
