About The Position

Job Overview Performs duties involving the healthcare, nutrition, and safety of clients within Head Start and Aging Services. This position will support staff with children’s health services or the well-being of participants at center locations.

Requirements

  • Possession of a valid State of Florida Practical Nurse License
  • Possession of a valid Florida Driver’s License
  • Possession of a valid CPR Certification
  • One (1) year of staff supervisory experience in a social services or health care services setting
  • One (1) year of experience in providing services to persons who have dementia
  • Knowledge in health and wellness education, CPR, First Aid, and AED.
  • Knowledge of departmental policies and procedures.
  • Knowledge of basic office practices and procedures.
  • Knowledge of customer service practices and procedures.
  • Knowledge of State of Florida licensure requirements.
  • Knowledge of medications, drugs, and narcotics and their effects.
  • Knowledge of nursing theory, techniques, and practices.
  • Knowledge of general pediatric nursing care.
  • Skill in organizing and planning activities.
  • Ability to assess health and wellness program needs and prepare corresponding programs.
  • Ability to schedule, coordinate, and conduct health and wellness programs for participants.
  • Ability to communicate effectively both orally and in writing.
  • Skill in performing emergency cardiopulmonary resuscitation and first aid.
  • Skill in the use of medical equipment.
  • Ability to work under stressful conditions.
  • Ability to work evenings, night shifts including weekends and holidays.
  • Ability to follow established procedures.
  • Ability to collect, organize and evaluate data and develop logical conclusions.
  • Ability to use a computer and related software.
  • Physical effort involving standing, sitting, or walking.
  • Ability to bend, stoop, kneel, stretch, and reach.

Responsibilities

  • Assists registered nurses with medical care.
  • Reviews and monitors medical chart updates for treatment progress, diverse reactions, and side effects.
  • Assists with administering medications as directed by physicians.
  • Records and reports in medical charts body temperature, blood pressure, pulse, weight, appetite, behavior, toileting, and use of assistive devices and reports adverse reactions to supervisor.
  • Cleans and dresses wounds.
  • Assists with the coordination and facilitation of wellness presentations and support groups.
  • Works as part of the interdisciplinary team to assist with care plans as appropriate.
  • Provides nursing assessments within their scope of practice.
  • May be required to oversee operation of the facility, staff, and clients on a limited basis.
  • Performs other related duties as required.
  • Ensure client safety by observing and participating in activities planned to maintain and enhance client’s health.
  • Observing for changes in conditions physically and mentally and communicating these concerns with team and caregivers in a timely manner.
  • Assists with supervisory role at the center; supports staff and nursing assistants.
  • Ensures center is within established participant to staff compliance ratio.
  • Follows food management guidelines provided though Nutrition.
  • Ensures safe self-administration of prescribed medications and renders assistance in medication administration when necessary.
  • Document medication administration promptly and accurately while maintaining up to date Medication Administration Record (MAR).
  • Maintains safe environment.
  • Provide supervision to all participants.
  • Reports adverse incidents/occurrences to RN and appropriate personnel.
  • Maintains bio-hazardous book and documents monthly waste if generated.
  • Maintains fall risk and mobility status for participants.
  • Provides fall risk prevention year-round.
  • Knowledgeable about use of durable medical equipment to include use of canes, wheelchairs, and walkers.
  • Adept at proper lifting techniques to prevent injury.
  • Active participant in Quarterly Care Plan Assessments.
  • The Licensed Practical Nurse (LPN) provides documented medical changes, concerns, and suggestions to the team.
  • Adhere to all regulations with AHCA ASPEN State Regulations Adult Day Care.
  • Required to be present at center a minimum of 75% of the time.
  • Completes required training by Hillsborough County and Licensed Practical Nurse (LPN) license requirements in timely manner as required.
  • Uses creativity to develop and present medical activities with a focus on health awareness, monitoring and education.
  • Work, in conjunction with the health Team and all other Head Start Staff, toward the goal of optimum health for children and to help link families into an ongoing health/mental health care source and encourage them to assume responsibility for their own care.
  • Observe daily health status of the children, following up on problems noted with staff, parents, and professionals.
  • Utilize RN for technical assistance.
  • Perform required health screenings and maintain current physical and dental reports.
  • After reviewing medical reports assist families in obtaining any identified medical care needed.
  • Serve as an advocate for child health.
  • Assist teachers in the preparation of children for various health procedures.
  • Responsible for maintaining current health records for designated center(s).
  • Teach positive health practices to staff, children, and parents.
  • Promote environmental health and safety in the center.
  • Perform home visits and recruitment as needed.
  • Perform other duties as they relate to the team-approach of the Head Start/Early Head Start philosophy.
  • Maintain confidentiality of all family and child records and all matters relating to families.
  • Adhere to relevant health and safety policies and procedures.
  • Report incidents or concerns of child abuse and neglect.
