A Path of Care Preferred Pathways of Hugo, OK is looking to hire a part-time ADvantage Case Manager for the Altus and surrounding area. Requirements

• Bachelor's degree and one year paid professional experience with aging or disabled population
• Valid driver's license and a good driving record
• Reliable transportation and proof of insurance
• Ability to travel long distances

Nice To Haves

• Oklahoma RN or LPN license preferred
• CPR certification within 60 days of hire (for RN/LPN only)
• Experience with the Advantage Program

Responsibilities

• Complete initial assessments of the member's needs
• Develop plans of care
• Perform monthly monitoring
• Initiate appropriate preventive interventions
• Coordinate services to keep members safe and independent in their home
• Participate in in-service programs and Advantage training
