About The Position

Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced Nurse Practitioner or Physician Assistant to join our Primary Care team in a unique Full-Time role. This position is ideal for an APP who thrives in a collaborative environment, values high-quality patient care, and enjoys supporting a physician-led team while maintaining a balanced and rewarding clinical practice. In this role, you will spend approximately 50% of your time providing direct patient care and 50% supporting the provider team through clinical coordination and workflow support. Unlike traditional primary care roles, you will not be responsible for building or managing your own patient panel, allowing you to focus on excellent care, teamwork, and clinical excellence. You will help coordinate high-quality, patient-centered care while supporting practice efficiency, population health initiatives, and quality improvement efforts. You will provide comprehensive evaluation and management services to adult and geriatric patients, grounded in evidence-based medicine and aligned with national quality standards such as Patient-Centered Medical Home (PCMH) principles.

Requirements

  • Nurse Practitioner: Master’s degree from an accredited nursing program
  • Physician Assistant: Graduation from an accredited PA program
  • Active Maryland license (or ability to obtain)
  • Current DEA license and Maryland Controlled Dangerous Substance (CDS) license
  • Current BLS certification (American Heart Association or approved provider)
  • Unrestricted license with no history of suspension or revocation
  • Eligible for participation with all payers
  • Strong clinical judgment and commitment to evidence-based practice
  • Proficiency with electronic medical records (EMR experience preferred)
  • Excellent organizational, time-management, and multitasking abilities
  • Strong interpersonal, communication, and problem-solving skills
  • Ability to work independently while thriving in a team-based environment

Nice To Haves

  • Prior experience in adult primary care preferred

Responsibilities

  • Provide high-quality, evidence-based care to adult and geriatric patients, including history taking, physical exams, diagnosis, treatment planning, and patient education
  • Spend approximately 50% of time in direct patient care and 50% supporting physician colleagues and clinical workflows (no independent patient panel)
  • Order and interpret appropriate labs, imaging, and referrals; perform procedures within scope of practice
  • Collaborate closely with physicians, nursing staff, care coordinators, and specialists to ensure coordinated, patient-centered care
  • Support care coordination activities, including follow-up on results, referrals, and chronic disease management
  • Document accurately and timely in the electronic medical record, ensuring appropriate coding and compliance
  • Participate in quality improvement initiatives, population health efforts, and practice workflow optimization
  • Maintain professionalism, regulatory compliance, and active participation in provider meetings and trainings
