About The Position

We’re looking for a Case Manager with a strong focus on Medicaid and financial services to support adults living with severe and persistent mental illness. This role is critical in helping clients access, maintain, and navigate Medicaid coverage, ensuring they receive the care and services they need. Requirements

  • Experience working with Medicaid, healthcare enrollment, or social services (strongly preferred)
  • Understanding of state Medicaid systems or application portals
  • At least 2 years of experience working with adults with severe and persistent mental illness
  • Strong communication skills—especially when discussing financial and healthcare-related topics
  • Ability to navigate complex systems and explain them in a clear, supportive way
  • Strong organizational and documentation skills
  • High school diploma required; Bachelor’s degree or equivalent experience preferred
  • Must be 21 years or older
  • Valid Kansas driver’s license and ability to transport clients
  • Ability to pass required background checks (KBI, DCF, Central Registry)
  • Must obtain and maintain Kansas Case Manager Certification (and CEUs as required)

Responsibilities

  • Assist clients with Medicaid applications, renewals, and redeterminations
  • Verify eligibility by collecting and reviewing financial and personal documentation (income, residency, disability status, etc.)
  • Submit and track applications through state Medicaid portals
  • Communicate directly with Medicaid agencies to resolve issues or delays
  • Educate clients on benefits, coverage options, and financial eligibility requirements
  • Ensure compliance with state and federal Medicaid regulations
  • Advocate for clients facing barriers to coverage or approval
  • Provide ongoing care coordination services for adult clients
  • Develop and maintain supportive, professional relationships
  • Facilitate treatment planning and coordinate care with multidisciplinary teams
  • Support clients in building independence, stability, and community integration
  • Provide crisis intervention in collaboration with the care team
  • Maintain accurate, timely documentation (90% completed within 72 hours)
  • Serve as a primary point of contact for Medicaid-related needs
  • Participate in team meetings, supervision, and care coordination sessions
  • Manage scheduling and prioritize client needs effectively
