The Opportunity Alliance is seeking compassionate, organized, and dedicated Care Coordinators ( Case Managers) to join our team-based case management model called Behavioral Health Home and make a meaningful impact in the lives of our clients. This is an exciting opportunity to work closely with individuals and families, connecting them to vital resources, advocating on their behalf, and supporting their journey toward stability and self-sufficiency. As part of the team, you will be working alongside a peer, nurse care manager, and medical/psychiatric consultants to provide holistic care coordination for adults with mental health and possibly medical needs. Care coordinators meet one on one with clients to assess and support their needs, while also having the support of our integrated care team.
