About The Position

AMHC is seeking a compassionate, organized, and motivated Full Time Adult Behavioral Health Home Coordinator / Community Integration Specialist to support adults living with long-term psychiatric disabilities in achieving stability, independence, and wellness within their communities.

  • Flexibility to meet client needs, including occasional extended hours
  • Commitment to confidentiality, ethics, and professional standards
  • The Adult Behavioral Health Home Coordinator/Community Integration Counselor requires an associate degree or equivalent credits in a Human Services field.
  • Must be certified at the full MHRT-C or provisional level.
  • Use of personal vehicle and active driver’s license is required.

  • Bachelor’s degree preferred.

Responsibilities

  • Build strong therapeutic relationships, including engaging hard-to-reach individuals
  • Assist clients in identifying personal wellness goals and developing skills to achieve them
  • Coordinate and manage services related to each client’s Plan of Care (Section 92) or Individualized Support Plan (Section 17)
  • Provide supportive counseling, decision-making assistance, and crisis intervention as needed
  • Develop and facilitate skill-building and psychoeducational groups
  • Coordinate care with physical and behavioral health providers
  • Assist clients in accessing community resources, including accompanying clients when appropriate
  • Support families and significant others (with client authorization)
  • Maintain accurate clinical and administrative documentation
  • Participate in team meetings, supervision, and staff development activities
