Be the First Line of Welcome & Protection in Resident Care As the Admissions & Infection Prevention Nurse, you will coordinate the full admissions process into Health Care, guiding residents and their families from their first inquiry through admission. You will assist with marketing efforts and provide tours, helping create a positive and informed first impression. You will also oversee the organization’s Infection Control Program, serving as the Infection Preventionist. This includes monitoring infection prevention practices, ensuring compliance with regulations, and promoting a culture of safety throughout the organization. Requirements

  • Active, unrestricted Registered Nurse (RN) license in Pennsylvania
  • Pennsylvania Infection Prevention/Control certification, or willingness to successfully complete the PA state-approved course upon hire
  • Proficient in Microsoft Office (Word, Excel) and experienced with Electronic Health Record (EHR) systems
  • Highly organized with strong time management and attention to detail
  • Safety First: You consistently follow universal precautions, including the proper use of personal protective equipment (PPE) to minimize risk and maintain a safe environment

Responsibilities

  • Lead and coordinate all Health Care admissions, including clinical and financial review, payer verification, authorizations, paperwork completion, EHR documentation, and census management.
  • Collaborate with the Director of Nursing, IL Administrator, therapy team, and RNAC to review referrals, insurance coverage, and ongoing care needs.
  • Serve as primary liaison to hospitals, discharge planners, physicians, managed care organizations, and other referral sources.
  • Oversee post-discharge follow-up to reduce readmissions and support continuity of care.
  • Maintain accurate admission logs, medical records, HIPAA documentation, credentialing files, and required regulatory reporting.
  • Lead the Infection Control Program as Infection Preventionist, including surveillance, outbreak investigation, vaccination tracking, regulatory compliance (including Act 52 and PA-PSRS reporting), and QAPI reporting.
  • Conduct audits, contribute to Quality Assurance and Process Improvement initiatives, and support clinical monitoring activities.
  • Participate in nursing on-call rotation and support overall nursing leadership operations.
