About The Position

The Admissions Coordinator is responsible for managing the resident admission process for our skilled nursing facility. This role works closely with hospitals, rehabilitation centers, physicians, case managers, and referral sources to ensure smooth transitions for residents. The position requires strong relationship-building, marketing, and customer service skills, along with a thorough understanding of long-term care regulations and admission procedures.

Requirements

  • Nursing license (RN or LPN) highly preferred.
  • Experience in skilled nursing, long-term care, admissions, healthcare marketing, or social services is strongly desired.
  • Knowledge of Medicare, Medicaid, managed care, and private insurance requirements for admission.
  • Excellent communication, customer service, and relationship management skills.
  • Ability to work independently while managing multiple priorities.

Responsibilities

  • Coordinate and oversee the admissions process from initial inquiry to move-in.
  • Screen and review healthcare referrals to determine clinical and financial eligibility.
  • Communicate with residents, families, hospital discharge planners, and healthcare providers to ensure a smooth transition into care.
  • Maintain compliance with state and federal regulations for SNF admissions.
  • Ensure all admission documentation is accurate, complete, and in accordance with CMS guidelines.
  • Maintain confidentiality of resident information in compliance with HIPAA.
  • Implement marketing and census development strategies to increase occupancy.
  • Develop and maintain relationships with hospitals, rehabilitation facilities, physicians, social workers, and case managers.
  • Attend and represent the facility at healthcare networking events, community outreach programs, and professional associations.
  • Deliver presentations to community organizations about skilled nursing, rehabilitation, long-term care, and aging services.
  • Monitor competitor activity and provide market insight to facility leadership.
