About The Position

Works with Facility Marketing Staff and Nursing to Coordinate the admission of Health Care Residents in Skilled Nursing Facility. Initiates contact with insurance companies and coordinates payment for HMO, PPO and Traditional insurance companies. Handles issues with admissions that involve clinical, social services and insurance/billing staff throughout the organization.

Responsibilities

  • Maintain an accurate facility daily list of available beds; may assist with preparing the facility daily census.
  • Conduct daily rounds checking to ensure available rooms are ready for new admissions
  • Prepare welcome packet for all new admissions
  • Tour potential clients and provide information about the facility and services provided.
  • Interview potential Residents/families in order to collect accurate data on admission inquiries.
  • Track pre-admission referral activity and pre-admission information daily and enter into computer.
  • Assist Nurse Liaison with the arrangement for and confirmation of all admissions.
  • Verify payer source and secure timely primary insurance approval and provide secondary insurance information to the Business Office for approval.
  • Communicate Clinical admission data to nursing and finance data to the Business Office for approval; transmit information to medical records for coding.
  • Obtain Insurance authorization and Level of Care for non-Medicare/insurance Residents.
  • Secure and forward Case Manager Identification information to Social Services.
  • Provides information to the Pharmacy for the purpose of preparing a cost out of medications for all referrals
  • Notify all departments when a Resident is scheduled for admission.
  • Notify Central Supply/Nursing/Rehab to secure necessary specialty equipment/supplies for new Resident
  • Explain facility philosophy, admission agreement, and attachments, to Resident/Responsible Party/Family
  • Prepare and distribute admission paperwork and Notice of Admission Form to nursing unit prior to admission.
  • Ensure that new admissions have a completed PASRR on record.
  • Complete all admission/re-admission packet information obtaining required signatures on all admission forms.
  • Prepares Resident identification wrist bands, Resident Room name plate, etc.
  • Whenever possible, escort the new Resident/family to the assigned nursing unit/room and introduce to staff.
  • Conduct several follow up visits with every new admission to ensure that both the Resident and their family are adjusting and satisfied with the care and services provided. Follow up with the appropriate Department Head to resolve any issues of concern.
  • Establish and maintain an excellent rapport with all referral sources and transferring institutions.
  • Assists Nurse Liaison in marketing the facility to reach and exceed budgeted census level.
  • Complies with all admission, transfer, and discharge policies and procedures.
  • Completes records in accordance with company policy, State and Federal guidelines.
  • Will act for Nurse Liaison during periods of absence, conducting hospital evaluations, reviews, etc.
  • Perform other duties as assigned.
