About The Position
Agawam West Rehab & Nursing is seeking an experienced Admissions Coordinator to join our team full-time. This role is ideal for someone with a strong understanding of skilled nursing or post-acute care who can ensure a smooth, professional admissions experience for residents and families.
Requirements
- Experience in a skilled nursing or healthcare setting preferred
- Familiarity with post-acute, rehabilitation, or sub-acute care environments
- Strong communication and customer service skills
- Highly organized with attention to detail
- Comfortable balancing administrative and interpersonal responsibilities
Nice To Haves
- CNA certification or prior CNA experience preferred, but not required
Responsibilities
- Coordinate the admissions process from referral through resident arrival
- Serve as a liaison between clinical teams, families, and referral sources
- Meet with residents and families upon admission to complete required documentation
- Collect and verify insurance and admission-related information
- Respond to inquiries in a timely, courteous, and professional manner
- Maintain accurate records and enter data into facility systems
- Manage incoming calls and emails related to admissions
