About The Position

The Admissions Care Manager serves as a key entry point to services, ensuring timely, equitable, and clinically appropriate access to behavioral health care. This position conducts brief behavioral health screenings and risk assessments, determines client eligibility and acuity, and facilitates placement into appropriate programs or community-based services. Working closely with the Admissions Director and multidisciplinary teams, this position manages referral prioritization, tracks service authorizations, and maintains accurate documentation within the electronic health record. The Admissions Care Manager also partners with community agencies - Including the Rose Andom Center - to maintain a small caseload to provide care management, service planning, and ongoing coordination for clients with complex behavioral health and social needs. This role requires strong assessment skills, crisis triage capability, cultural humility, and a commitment to serving underserved and marginalized populations. The ideal candidate thrives in a fast-paced environment, communicates effectively with referral partners and families, and maintains a high standard of documentation, confidentiality, and customer service.

Requirements

  • A confirmed bachelor’s degree in a relevant field in Counseling, Social Work, or related field is required.
  • Valid driver’s license, auto insurance, willingness to travel to homes/communities.
  • Requires living in or near the location of the posting.

Nice To Haves

  • One year of relevant experience in Behavioral Health and/or Case Management is preferred.
  • Bilingual (Spanish) preferred

Responsibilities

  • Conducts brief behavioral health assessments and screening to determine eligibility, treatment needs, and treatment options – obtain and summarize information to document in the electronic health record.
  • Monitor Admissions phone line and conduct telephonic screenings
  • Assess risk factors including suicidality, homicidality, self-harm, substance withdrawal risk, and other safety concerns.
  • Enter basic enrollment and screening information for clients into the EHR. This includes any insurance information along with checking Medicaid eligibility.
  • Track service authorizations from referral sources.
  • Sets and manages client acuity and prioritization process/referrals in collaboration with Admissions Director.
  • Communicate and collaborate with team staff pertinent information when making client assignments.
  • Strong written and verbal communication skills, as well as exceptional customer service skills – includes responding to referrals in a timely manner.
  • Timely and thorough documentation of outreach attempts and screenings completed.
  • Maintain referral tracking spreadsheet.
  • Crisis intervention – evaluate need for incoming client to be immediately escalated to therapist for safety planning
  • Maintain client confidentiality.
  • Serve as liaison between referral sources, hospitals, primary care providers, social service agencies, and internal clinical teams.
  • Coordinate timely placement into appropriate internal programs.
  • Provide referrals to external providers when services fall outside the organization’s scope.
  • Ensure smooth handoffs to ongoing treatment teams.
  • Submit required documentation and updates to referral partners.
  • Partner with Rose Andom Center to manage referrals for clients.
  • Directs the client in arranging necessary community support services, such as mental health services, public assistance, vocational training, and similar.
  • Documents all direct and indirect contacts with the client, their family, and community support services.
  • Service plan development – specifying goals and actions to address member needs, ensuring participation, and identifying a course of action; includes transition plan development.
  • Submits Progress Summaries to the referring agencies with ongoing recommendations and makes weekly contact with these agencies.
  • Performs other duties as assigned.
  • Promote equitable access to care for underserved and marginalized populations.
  • Maintain knowledge of community resources including housing, employment, financial assistance, and social support services.
  • Engage individuals who may be ambivalent about treatment in a motivational and strengths-based manner.
  • Strong written and oral communication skills via phone and face-to-face.
  • Understand HIPAA, protected health information, and confidentiality.
  • Proven ability to work with a high level of independence, maintain confidentiality, prioritize assignments, and manage time effectively.
