The Administrative Support Assistant provides direct administrative and operational support to the Social Determinants of Health (SDOH) Social Workers. This role supports both inpatient and ambulatory patients to ensure effective community resource linkage and smooth transitions of care. The assistant facilitates communication, documentation, and follow-up activities that help address barriers impacting patients' health and overall well-being.This position will report to the Casae Management Director.
