Case management services are designed to assist patient and family members with continuing care arrangements that may be necessary following discharge from the hospital. Case managers work with physicians, nurses, physical therapists and other members of the health care team to identify and coordinate alternative care or services that will assist in recovery. The Unit Case Management Department’s Assistant (CMA) supports Case Management (CM) in coordination of case CM services. The CMA helps facilitate coordination and collaboration of CM services through timely responses and redirection of concerns throughout the daily operations to meet quality effective patience care and enhance the patient-care experience with CM and Baptist Health Lexington (BHL). The CMA strives for excellence in service by building a collaborative work environment among all staff and departments.
