About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Community Bridges, Inc. (CB I) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models. Job Summary The F/ACT Specialist is a senior F/ACT staff with a wide range of F/ACT and/or rehabilitative experience. The F/ACT Specialist will perform the duties of the Clinical Coordinator in the absence of the Clinical Coordinator.

Requirements

  • Highschool diploma or GED required.
  • Associates Degree in a field related to behavioral health preferred.
  • 1 year of experience working on an F/ACT team preferred.
  • 1 year of experience working with the SMI population required.
  • Additional experience in multiple settings and levels of care, including both mental health and substance abuse preferred.
  • Ability to pass criminal background check for jail visitation/clearance.
  • Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred. Or a combination of education and experience.
  • Current AZ Driver’s License.
  • Clean 39-month MVR.
  • Fingerprint Clearance Card required.
  • Behavioral Health Technician (BHT)
  • Trained to complete the Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDAT)
  • Peer Certification (if applicable)
