About The Position

Adapt Integrated Health Care is looking for a qualified ACT Therapist to join their team in Roseburg, Oregon! The Assertive Community Treatment Therapist provides integrated outpatient mental health treatment services that are based on medical necessity and that support and promote mental health and recovery. Their responsibilities are to engage and respond in crisis intervention; facilitate access for mental health treatment; provide therapeutic interventions including informed consent, advocacy, and integrated care; assist with access to community resources; facilitate development and retention of natural and community supports; provide appropriate individual, group, and community psycho-education; actively work to reduce stigma, barriers, and to build a community foundation for success. Primary assignment to ACT.

Requirements

  • Master's degree in psychology, social work or related field with experience in a mental health setting.
  • Licensed providers preferred.
  • Current MHACBO eligibility/registration as a QMHP or current Oregon Licensing Board Associate Registration
  • Must possess a valid Oregon Driver's License.
  • Ability to communicate effectively in both written and oral form. This pertains to both communications with clients, coworkers, and community partners as well as documentation.
  • Knowledge of psychological theory as well as principles and practices of mental health treatment, especially those pertaining to crisis intervention.
  • Strong planning and time management skills, including being flexible and adaptive.

Responsibilities

  • Provides community-based treatment services through assessments and evaluations, individual and group treatment and supports through prescription of outpatient treatment services.
  • Collaborates with clientele and in consultation with team; completes Safety and Recovery Plans for those individuals that are at risk.
  • Performs screenings for referrals that assess the level of SPMI clientele and their appropriateness for the Assertive Community Treatment (ACT) Program.
  • Under the direction of the ACT Lead, coordinates client admission process and treatment, rehabilitation and support services, and schedules admission interviews.
  • Develops and coordinates the initial assessment and initial treatment plan and provides clinical support in the development and implementation of the treatment plan for clients.
  • Consults, refers, and coordinates with agency partners and providers; coordinates discharge planning that includes supports and active planning for provision of continuity of care.
  • Performs many functions and treatment interventions to ACT clients including crisis intervention, eligibility, access, and enrollment services.
  • Provides individual and/or group therapy, provides case management services that enables clientele to obtain needed community and treatment services (e.g., substance use, medical); provides consultation and training for and with clients, staff, professionals, other services agencies and the public as needed.
  • Participates in administrative tasks related to meeting fidelity, such as documenting and tracking fidelity components.
  • Helps coordinate and organize community and agency education components and contributes to developing ideas and strategies to improve the program.
  • Other duties as assigned.
