Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served. Outreach Counselors act as a liaison between Persons served, community agencies, and families to coordinate treatment including employment and education entities, emergency service providers, outpatient clinics and others. They are committed to cultivating a mutually respectful partnership, setting appropriate limits, and maintaining a helping role with individuals served and family members. Outreach Counselors encourage and empower individuals to see beyond and live beyond perceived limitations by providing them with the tools required for success!
