Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served. The Deaf and Hard of Hearing Counselor will provide support to people who are either deaf or hard of hearing and are diagnosed with a mental illness and/or substance use disorder and live on their own, or with families, in the community. They will act as a liaison between Persons served, community agencies, and families to coordinate treatment including employment and education entities, Care Coordination Entities, Emergency Service Providers, and outpatient clinics. Outreach Counselors encourage and empower individuals to see beyond and live beyond perceived limitations by providing them with the tools required for success.
