About The Position

The company is seeking Hospice Liaisons, also referred to as Account Executives, who are adept at coordinating, organizing, and making connections, and are committed to serving patients with compassion and integrity. This role involves coordinating patient referrals, establishing relationships with local hospitals to secure referrals, and educating the community and referral sources about hospice care. The liaison works closely with patients and their care team to ensure continuity of care, proper communication, and correct documentation. Team members, including hospice liaisons, receive support from leadership and professional development opportunities with room for advancement. Requirements

  • A heart to serve patients and families and a passion for providing the best possible care
  • Minimum of an Associate Degree required in field of study or proven work experience in health related field
  • 2 year of sales experience in a clinical care setting
  • Reliable transportation
  • Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively

Nice To Haves

  • Hospice sales experience preferred

Responsibilities

  • Coordinating & generating new hospice business in both existing and new accounts
  • Identifying new markets and maintaining existing service
  • Coordinating care for referred patients
  • Serving as a liaison between the agency, hospitals, medical community and other referral sources
  • Educating the community and the medical profession/referral sources regarding hospice services
  • Daily interaction with patients, medical professionals, other referral sources, and the community to assure continuity of care and to coordinate appropriate communication and documentation
  • Working closely with agency personnel to ensure that patient care is handled efficiently and effectively
  • Regularly engaged away from the employee’s place of business in performing these duties
