About The Position

The Youth Rapid Rehousing Case Manager provides direct support to youth ages 18–24 experiencing homelessness, with the goal of securing and sustaining safe, stable housing. This position supports participants through housing search and placement, mediation with landlords, individualized service planning, and connection to community-based resources. The Case Manager ensures that participants build independent living skills and achieve long-term housing stability, while working from a trauma-informed, strengths-based, and equity-centered approach. The role involves both office-based responsibilities and fieldwork, including home visits and community outreach.

Requirements

  • Education: Bachelor’s degree in Social Work, Human Services, or a related field preferred.
  • Experience: Minimum of three (3) years working with homeless youth, families, veterans, or underserved populations preferred.
  • Knowledge: Familiarity with housing assistance programs, applicable housing laws, and community-based service systems.
  • Skills & Abilities: Strong case management, crisis intervention, and advocacy skills.
  • Knowledge of trauma-informed care, mental health, substance use, disability, domestic violence, and LGBTQ+ supports.
  • Excellent communication (verbal and written), interpersonal, and conflict resolution skills.
  • Ability to work independently, manage a caseload, and build effective partnerships with landlords and agencies.
  • High level of integrity, accountability, and time management.
  • Proficiency in Microsoft Office Suite, especially Outlook.
  • Requirements: Must pass a criminal background check and pre-employment drug screening.
  • Valid driver’s license, automobile insurance, and reliable transportation required.
  • Ability to bend, stoop, walk, sit, stand, and lift up to 25 lbs.
  • Availability to travel locally for meetings, training, and home visits.

Responsibilities

  • Housing & Case Management
  • Conduct comprehensive assessments to identify housing barriers and individual needs.
  • Assist participants in locating, applying for, and securing housing of their choice.
  • Provide mediation, advocacy, and problem-solving with landlords to promote housing retention.
  • Educate participants on lease agreements, housing rights, and responsibilities.
  • Conduct regular home visits and follow-ups (in-person, virtual, or phone) to monitor stability and progress.
  • Revise service and housing goal plans regularly to reflect progress toward independence.
  • Resource Navigation & Community Partnerships
  • Develop strength-based, solution-focused service plans with participants.
  • Connect participants to supportive services such as healthcare, education, employment, benefits, and transportation.
  • Build and maintain collaborative relationships with housing providers (e.g., Section 8, private landlords), social service agencies, and community partners.
  • Support participants’ reintegration into the community through advocacy and wraparound resources.
  • Monitoring, Documentation & Compliance
  • Maintain accurate and timely case records in HMIS and internal databases.
  • Complete intake, case notes, service tracking, annual assessments, income changes, and inspection reports for all new and current housing units.
  • Conduct home inspections to ensure compliance with housing quality standards and funder requirements.
  • Participate in agency and community meetings, trainings, and collaborative partnerships.
  • Prepare reports as required by grants, funders, and agency leadership.
  • Team & Organizational Support
  • Collaborate with the Housing Program Manager and team to ensure program goals are met.
  • Participate in weekly supervision, providing updates on caseload and program activities.
  • Work cross-departmentally to improve housing services and ensure holistic participant support.
  • Treat all participants, team members, and volunteers with dignity, care, and respect while modeling reStart’s mission and values.
