Case Manager - Transitional Housing & Rapid Rehousing
About The Position
The YHDP (Youth Homelessness Demonstration Project) Transitional and Rapid Rehousing Case Manager provides case management services to homeless and formerly homeless young adults (18-24) utilizing the Transition in Place housing model to facilitate rapid exit from shelter. The YHDP program uses evidenced based best practices such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, and Housing First. The case manager will work with participants to recertify program eligibility, conduct home visits, link to long-term supports, and follow up to assist in maintaining their housing and independence. This is a non-exempt, union position with a pay rate of $19.00 per hour. The hours of work are Monday through Friday, 8:30am – 5:00pm.
Requirements
- Bachelor’s degree in a social work, counseling, or social service field
- Lived experience of homelessness and/or housing instability and embodiment of the diverse identities of young adults who experience homelessness in Cuyahoga County they serve, particularly, but not limited to, BIPOC, LGBTQIA+, and Young Adult
- Valid Ohio driver’s license with less than six points and proof of automobile insurance, adhering to the minimum requirements of Ohio’s Financial Responsibility Act
