About The Position

Summary: The Life Coach/Case Manager will have extensive experience working with lesbian, gay, bisexual, transgender, gender variant youth and their families. This position will assist young peop l e transition into adulthood and connect to services in the area of housing, education, health, life - skills, relationships and more . Knowledge of community resources and their qualifications is critical.

Requirements

• One year of office experience required
• Strong organizational skills required
• Group facilitation skills required
• Strong communication skills (written and verbal)
• Experience with Microsoft Office Suite, Word, Excel, Outlook, Google Docs
• Able to handle many varied tasks at one time
• Able to prioritize tasks and self-motivate
• Experience working with diverse populations, various backgrounds and cultures
• Must have the ability to work independently and within small groups

Nice To Haves

• Prefer experience working with at - risk and LGBTQ teenagers

Responsibilities

• One-on - one life - coaching/case management with LGBTQ Youth & Families
• Timely and through assessments, case planning and ongoing updates quarterly and as clients progress
• Timely and accurate documentation of case notes
• Linkage to services such as therapy, housing, education, life-skills groups and more
• Regular in-vivo teaching which may include riding the bus with youth, grocery shopping, visiting college campuses, viewing apartments, teaching youth how to cook and more
• Participation in regular case conferences with therapist as well as case-managers/peer support specialists
• Develop, implement & facilitate curricu l um for Life Skills group as well as coordina t e guest trainers/speakers
• Other duties as assigned
