The Case Manager provides community-based case management and rehabilitative support to individuals with mental health and/or substance use disorders. This role promotes recovery, independence, stability, safety, and community integration through assessment, service planning, skill development, crisis support, advocacy, and coordinated care. Case Managers partner with clients, families, treatment team members, and community providers to identify needs, strengths, goals, and barriers. Clients served may include adults or youth enrolled in Comprehensive Community Services (CCS), Community Support Program (CSP), Children’s Long-Term Support (CLST), or the Coordinated Services Team (CST).
