What You Will DO Work with consumers who have a mental health diagnosis whose severity is impairing their ability to be successful in home, school, work or their community. Provide services which will build skills allowing the consumers to achieve success. Work collaboratively with the consumer and their treatment team to achieve goals as prescribed by the person centered plan.

Requirements

  • High School diploma or equivalent.
  • 21 years-of-age or older
  • Strong oral and written communication skills using the English language.
  • Basic personal computer skills which enable the entry, retrieval and use of electronic data.
  • A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.
  • CSPs in some regions may be required to provide crisis services, and therefore will be required to work rotating on-call shifts.

Nice To Haves

  • Post-secondary education in a human services field and/or related work experience.
  • Experience with Microsoft Office software.

Responsibilities

  • Review client person centered plans and develop activities to enhance functioning levels in identified areas of need.
  • Provide direct services in location most appropriate based on client need and document progress in each person’s chart. Locations may include the home, school, work, or other community settings.
  • Provide one-to-one support to clients who are in crisis or who need a constant presence in order to remain stable and avoid a more restrictive placement.
  • Collaborate with family members and other members of the treatment team in identifying goals and strategies which reinforce desired behavior.
  • Responsible for the implementation of scheduled day activities such as exercise, socialization, personal goal setting, illness management and community outings as appropriate.
  • Review client incidents with supervisor and submit written report as outlined in policy and procedure.
  • Advocate for client rights and community integration. Preserve dignity and confidentiality of each person on caseload.
  • Attend and actively participate in clinical supervision and program staffing. Provide input to supervisors and other treatment team members regarding clinical and program issues.
  • Drive to community settings for the delivery of services and related program needs. This includes transporting the client in a personal or company vehicle.
  • Meet with the client/guardian where service is needed. This may include but is not limited to the home, school, work, store, community location.
  • Meet administrative and clinical documentation requirements both in terms of content and timeliness as outlined in agency policy.
  • Meet client care hour expectations as outlined in agency policy.
  • Enter and maintain clinical schedule in the electronic health record scheduler.
  • Maintain and advance knowledge and skills related specifically to position or to field of work through regular review of pertinent materials or participation in training events as approved by supervisor.
  • Adhere to the standards, policies and instructions established by PMHS Policy and Procedure and any applicable ethical codes within field of employment.
  • Review and comply with the Pawnee Code of Conduct outlined in the Employee Handbook.
  • Monitor and assist with medication administration compliance as needed.
  • Perform verbal crisis de-escalation and, if necessary, physical behavior management interventions (such as therapeutic holds) using Pawnee’s approved crisis de-escalation program to maintain environmental safety for the consumer and others.
  • Perform all other reasonably related duties as assigned by immediate supervisor or other management personnel.
